By signing this form, I acknowledge and agree that the digital copy of my Criminal Record Check submitted to the Doula Association of Alberta (DAA) is a valid and accurate representation of the original document.
I agree to retain the original physical document and understand that the DAA may request the original document for verification or submission at any time.
The submitted Criminal Record Check will remain valid for two years from the date of issue, after which an updated Criminal Record Check must be submitted to the DAA.
